Important: If you already have a Lifecard account and need to complete an admission for a family member (such as your child or partner), please follow these steps:

Log in to your existing Lifecard account.
When you’re asked to choose a Lifecard account, select Create a sub account.
Follow the prompts to set up a new profile for your family member.

This ensures their details are kept separate from yours and prevents your information from being overwritten.

What you’ll need before starting:

Medicare and/or benefits card
(e.g. Medicare, Pension or Healthcare card)
Private health or payment details
(e.g. Private Health Fund, DVA or Workcover)
Current medications
(A list of the medications you take, including dosage and frequency)
Next of kin or emergency contact
(Plus any admission information from your doctor, such as your admission date and reason)
